Fill out the submission questionnaire while you are online and simply click the "Submit" button when you are finished to instantly and SECURELY submit your data to us. Option b.)
The download window will open automatically. Save the questionnaire into a file on your hard drive where you will remember where it is located. When you double click on the saved file it will automatically extract and open as a .html file which will allow you to either open it and fill it out in your browser at your convenience, or print it out and fill it out by hand. If you fill it out in your browser, be sure to print it out before closing the browser, or you will lose all the data you input into the questionnaire.

| Upon receipt of your submission questionnaire we will prepare your bankruptcy petition, all schedules, Summary of Schedules, Statement of Financial Affairs, mailing matrix, and all required declarations and verifications in accordance with the rules of your local bankruptcy court.
Depending upon your selection of services in our submission form, we will either forward your original prepared, printed, documents to you by Priority Mail, Express Mail; or electronically, by Internet download, for you to print out of your own computer. You should sign an original set of the documents in the places indicated on the documents, then run additional copies of the signed documents for submission to your bankruptcy court. We will provide you with all the necessary contact telephone information and mailing addresses for your local bankruptcy court. |

| You should contact your local bankruptcy court and ascertain how many copies of your documents your local court requires. Then either mail or take all the required documents to your bankruptcy court along with a money order made out to the "Clerk of the Bankruptcy Court" (bankruptcy courts will not usually accept personal checks).
